AMIST Income Protection is supplied by Australian Income Protection Pty Ltd and claims must be made directly to their claims department.

Step 1
Contact the claims department at Australian Income Protection on

Alternatively you may download a claim form here.
Step 2
Make sure all three (3) sections are filled out:
Section A (Claimant to complete),
Section B (Doctor to complete) and,
Section C (Employer to complete).
Step 3
Ensure all questions on the claim form are completed in full, if you have any questions please don't hesitate to contact the AIP claims department.
Step 4
Post original claim form and additional documentation to:
